Make sure that you have signed, dated, and returned all required documents that were given to you at the time you scheduled surgery. If you need these documents, you may download them by selecting the appropriate format and clicking the link below. Please print the documents out, then sign, date, and return the documents marked "FAX" via the following:

MEDICATIONS:
Tylenol ONLY!!
Discontinue ALL aspirin and ibuprofen products two (2) weeks prior to surgery
AND two (2) weeks after surgery! Do not use any Non-prescription products containing
aspirin and/or aspirin-like compund(s)! The following is a list of medications
to discontinue:
Alka-Seltzer Antacid/ Pain Reliever |
P-A-C Analgesic Tablets |
Also discontinue:
Separate Vitamin E
Diet Pills ( this does include Metabolife & Metabolite)
Any natural Herbs & Herbal Supplements
